From Prescription to Participation:
Building an Integrated Preventive Health Ecosystem
The success of preventive healthcare is increasingly shaped not only by what happens within hospitals and clinics, but by what happens when individuals return to their homes and communities. Turning clinical advice into sustained action requires stronger connections between healthcare, community resources, behavioural support, accessible programmes and technology.
But how do we make that connection work in practice?
Led by Dr Fadzil Hamzah, Senior Staff Physician, Department of Sport and Exercise Medicine at Changi General Hospital, this multidisciplinary conversation brings together perspectives from across Singapore’s preventive health ecosystem to explore the journey from prescription to participation.
Drawing on real-world experience, the discussion will examine how individuals can be identified and supported earlier, how clinical recommendations can be connected with appropriate community programmes and resources, and what helps people translate advice into meaningful and sustainable changes in their everyday lives.
The conversation will also look beyond individual interventions to the wider ecosystem required to make preventive health work: how healthcare and community partners can collaborate across traditional boundaries, where gaps remain, what enables sustained participation and behaviour change, and how technology can support more connected and person-centred models of care.

How can we better bridge the transition from clinical recommendation to community participation?
What helps individuals adopt and sustain healthier behaviours beyond the clinical setting?
How can healthcare providers, community organisations and other partners work more effectively together?
Where are the gaps between healthcare and community support, and what needs to change?
How can technology strengthen access, engagement and continuity of care without losing the human element?
What can we learn from real-world programmes and experiences that could be applied across different healthcare and community settings?
For healthcare and rehabilitation practitioners, community-care professionals, policymakers and other stakeholders from Singapore and across the region, the session offers practical perspectives and transferable lessons for strengthening the connections between clinical care and everyday life, improving health outcomes, supporting healthy ageing and enabling people to remain active and well within their communities.
